Your Role as a Hospice C.N.A /H.H.A.:

  • Verify the Plan of Care before each visit and provide care according to RN delegation, physician's orders, and established guidelines.
  • Incorporate patient care goals into nursing visits, as evidenced by documentation.
  • Communicate significant findings, problems, or changes in condition to the RN Case Manager, physician, or other relevant personnel.
  • Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, or physician.
  • Implement patient safety plans using available resources.
  • Assist patients in learning self-care techniques.
  • Suggest revisions to the plan of care and notify the RN Case Manager.
  • Provide specialized nursing skills under the direction of a Registered Nurse or physician.
  • Collaborate with the RN Case Manager for advice and instructions.
  • Document medication regimens and update medication profiles.
  • Treat patients and caregivers effectively and compassionately.
  • Implement actions to prevent avoidable hospitalizations and ER visits.
  • Provide positive and supportive communication to all stakeholders.

What We Require:

  • High School Diploma or GED equivalent.
  • 1 year of experience as a Hospice Aide or Nursing Assistant.
  • Completion of a state-established or other Hospice Aide training program meeting federal requirements, including a competency evaluation.
  • Home health environment experience preferred.
  • 50% travel required.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including patient transfers and equipment movement.
  • Valid driver's license, insurance, and reliable transportation.

You will report to the Clinical Team Manager.
