Traditional sleep care is fragmented and hard to access. Long wait times. Poor follow-up. Limited support for the patients who need it most.
Polaris Sleep is building a virtual model designed to make diagnosis and management of sleep apnea more scalable, payer-aligned, and accountable to outcomes. We're early-stage and moving quickly, and we're building the infrastructure to have a positive impact on our patients.
Role Summary
The Care Manager is the cornerstone of member experience for the Polaris Sleep virtual clinic. You will be the primary point of contact for members after diagnosis, owning PAP onboarding and education, adherence monitoring, barrier resolution, and long-term engagement that drives clinical outcomes and payer ROI.
We are looking for a Medical Assistant with meaningful remote patient monitoring experience and a clinical background in pulmonary, sleep, cardiology, or respiratory care.
This is a fully remote, patient-facing role for someone comfortable operating independently in a lean environment. The ideal candidate is comfortable energized by the idea of building a Care Management function and team from the ground up, and focused on providing a high touch, compassionate patient experience.
Key Responsibilities
- Conduct structured onboarding calls following OSA diagnosis, covering PAP education, therapy expectations, and insurance compliance requirements
- Monitor PAP adherence data via CPAP platforms (ResMed AirView, Philips EncoreAnywhere, or equivalent) and conduct proactive outreach when members are at risk of dropout or non-compliance
- Address common adherence barriers including mask fit, pressure intolerance, dryness, and motivation using motivational interviewing techniques
- Coordinate with DME partners on equipment adjustments, supply reorders, and prior authorizations; facilitate mask changes and device replacements as needed
- Escalate clinical concerns to the supervising APP or Medical Director and collaborate on members requiring therapy adjustments
- Maintain timely documentation of care management activities in support of RPM and CCM-adjacent billing
- Track member outcomes including adherence rates and AHI reduction, identify disengagement trends, and surface findings to the team
Required Qualifications
- Demonstrated RPM or chronic disease management experience in pulmonary, sleep, cardiology, or respiratory care, ideally in a telehealth or virtual care setting
- Strong patient communication skills across phone, video, and async channels; comfortable managing an independent panel with competing priorities
- Proficiency with remote monitoring platforms and EHR systems
Ideal Qualifications
- Direct experience with PAP therapy, CPAP platforms, or respiratory therapy
- Familiarity with RPM/CCM billing and DME compliance documentation
- Prior experience as a first hire in a Care Management or Customer Service role within a startup or small business environment

Compensation
$75,000 - $95,000 + Benefits and PTO
