Job Summary

The Reentry Care Manager leads care coordination for Managed Care enrollees, focusing on Reentry Targeted Case Management (rTCM) for individuals transitioning from carceral facilities into the community in Washington state.

Key Responsibilities

  • Develops, assesses, and facilitates complex care management activities, primarily for mental and behavioral health needs, to achieve high-quality, cost-effective healthcare outcomes.
  • Creates personalized care plans and provides education for members and their families regarding mental health and substance use disorders.
  • Evaluates member needs through phone or in-home visits, assessing available resources and recommending/facilitating care plans to achieve optimal outcomes, addressing behavioral health and social determinant needs.
  • Conducts outreach via telephonic, digital, home, and/or other site visits to assess member needs and collaborate with resources.
  • Develops ongoing care plans for members with high acuity, identifying necessary providers, specialists, and community resources, including those for mental health and substance use disorders.
  • Coordinates between members, families/caregivers, community resources, and the care provider team to ensure access to identified services.
  • Monitors care plans and member status/outcomes, tracking treatment side effects, complications, and clinical symptoms, providing recommendations based on identified needs.
  • Facilitates care coordination and collaborates with providers/specialists to ensure timely access to care and services.
  • Collects, documents, and maintains member information and care management activities for regulatory compliance.
  • Educates members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits, including behavioral health and social determinant needs.
  • Provides feedback to leadership on improving care delivery and quality for members in a cost-effective manner.

Required Qualifications

  • Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing.
  • 2-4 years of related experience.
  • WA State Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN with BH experience, as required by state contract.

Additional Skills & Qualifications

  • Direct experience with the reentry, justice-involved, or formerly incarcerated population is highly preferred.
  • Licensed BH professional is preferred; RN with behavioral health experience will suffice.
  • Experience in substance abuse counseling within healthcare is preferred.
