Position Overview

Centene is seeking a dedicated LTSS Service Care Coordinator to join our team and help transform the health of our communities. This is a hybrid role, requiring 50% remote work and 50% travel to visit members in Southern Illinois.

Service Area: Jackson, Williamson, Union, Johnson, Pope, Alexander, Pulaski, and Massac counties for the Medicaid MLTSS and Duals Aging and Physically Disabled Program.

Important: Applicants must reside in one of the specified counties. Mileage will be reimbursed at the current IRS guideline rate.

Work Schedule:

  • Training: Monday - Friday, 8:00 AM - 5:00 PM
  • Post-Training: Monday - Friday, 8:00 AM - 4:30 PM (with a 30-minute lunch break)

Key Responsibilities

This role focuses on developing, assessing, and coordinating holistic care management to achieve quality, cost-effective healthcare outcomes. You will assist in creating personalized service care plans and educate members and their families on available services and benefits.

  • Evaluate member needs and available resources to facilitate optimal outcomes.
  • Assist in developing long-term care plans and identifying necessary providers and community resources.
  • Coordinate between members, families, and care providers to ensure service accessibility.
  • Provide resource support for various member needs (e.g., employment, housing, independent living).
  • Monitor care plans, member status, and outcomes, making recommendations as needed.
  • Liaise with long-term care providers and partners to meet member needs.
  • Collect, document, and maintain member information and care management activities in compliance with regulations.
  • Conduct on-site visits to assess member needs and collaborate with providers.
  • Educate members and families on procedures, service options, referrals, and benefits.
  • Provide feedback to leadership on improving care quality and service delivery.
  • Perform other assigned duties and comply with all policies.

Qualifications

Required:

  • Bachelor's degree and 1 year of related experience.
  • OR equivalent experience demonstrating applicable knowledge and skills.

For Illinois Plan Only (Physically Disabled/Elderly): In addition to the above requirements, candidates must meet one of the following:

  1. RN licensed in Illinois.
  2. Bachelor's or Master's degree in a human services-related field (e.g., Human Services, Social Work, Psychology, Sociology, Rehabilitation Counseling).
  3. LPN with 1 year of experience in comprehensive assessments and service provision for the elderly.

Compensation and Benefits

Pay Range: $22.94 - $38.79 per hour

Actual pay is determined by skills, experience, education, and other job-related factors. Total compensation may include incentives. Benefits are subject to program eligibility.
