We achieve this through hands-on and authentic mentoring, guidance, training, networking, and providing other essential supports and referrals to needed services.
About the Role
The Re-entry Case Manager will play a critical role in supporting the successful reintegration of justice-impacted individuals. Reporting to the Chief Program Officer, the Case Manager will provide direct case management services to participants, addressing barriers to employment, housing, healthcare, education, and other essential areas. This role requires a trauma-informed, empathetic approach to service delivery and a deep understanding of the challenges faced by returning citizens.
What You'll Do
As the Re-entry case manager you will:
- Conduct Participant Intake & Assessment: Perform comprehensive intake assessments to understand participants' goals, strengths, barriers, and service needs.
- Develop Individualized Service Plans (ISPs): Co-create goal-oriented action plans with participants, focused on employment readiness, housing stability, mental health, substance use support, and personal development.
- Provide Case Management & Support: Deliver ongoing case management, including referrals, appointment scheduling, transportation coordination, and check-ins.
- Facilitate Resource Navigation: Assist participants in accessing internal and external services, including public benefits, workforce development programs, legal aid, and more.
- Track Progress & Maintain Documentation: Maintain accurate and timely documentation in accordance with organizational protocols and funder requirements.
- Engage in Crisis Intervention: Respond to urgent needs and provide short-term crisis support.
- Collaborate with Partners: Work closely with community providers, re-entry networks, housing organizations, and employers.
- Advocate for Participants: Serve as a strong advocate for participant needs within systems that include criminal justice, housing, employment, and healthcare.
- Support Program Development: Provide feedback and insights to help evolve programming based on participant needs and trends.
What We're Looking For

- You have prior experience in case management, social work, re-entry services, or a related field (lived experience is valued and welcomed).
- You are deeply committed to the dignity, rights, and empowerment of justice-involved individuals.
- You are highly organized and able to manage multiple caseloads while maintaining empathy and professionalism.
- You have strong verbal and written communication skills.
- You are familiar with Washington, D.C.-based resources and systems relevant to returning citizens.
- You are trauma-informed and culturally responsive.
- You are comfortable using case management databases and common software (Microsoft Office, Google Suite).
- Bilingual candidates and those with direct experience navigating re-entry are encouraged to apply.
What You Will Get
- A competitive salary of $57,000 per annum.
