Chronic Care Staffing is seeking a dedicated Remote Enrollment Coordinator to act as the primary point of contact for patients, guiding them through our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services to enhance their health outcomes. What Success Looks Like
Success in this role involves mastering the enrollment process and fostering patient trust. Key responsibilities include patient outreach, determining eligibility, and clearly communicating program benefits to secure consent.
Daily Responsibilities
- Conducting professional and compassionate phone outreach to patients regarding CCM and RPM programs.
- Assessing patient eligibility and clearly explaining program benefits and expectations.
- Accurately documenting verbal consent and patient interactions in the EMR, adhering strictly to CMS and HIPAA guidelines.
- Collaborating with internal care teams for seamless patient hand-offs and continuous support.
- Maintaining high documentation standards for accurate care planning and billing.
Long-term Success
Develop into a subject matter expert in enrollment workflows, contributing to increased program participation and improved patient engagement metrics. Independently manage daily queues, ensuring all patient interactions are helpful, informative, and compliant.
Requirements
Must-Haves:
- Active, nationally recognized Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification.
- Must reside in either MST or PST time zones.
- Bilingual fluency in English and Spanish.
- Current BLS certification.
- Direct, hands-on experience with CCM and/or RPM programs.
- Strong proficiency in Google Suite and electronic medical record (EMR) systems.
- Ability to work independently in a secure, HIPAA-compliant home office.
- Excellent verbal and written communication skills, with the ability to simplify complex information.
Nice-to-Haves:
- In-depth knowledge of CMS regulations and documentation requirements for chronic care.
- Previous experience in care coordination or transitional care.
- Familiarity with clinical quality measures and reporting.

- Pay Range: $17.00 - $20.00 per hour.
- Schedule: Full-time, with no weekend work required.
