About Safe Hands Watch

Safe Hands Watch partners with healthcare providers to improve patient outcomes through Chronic Care Management and Remote Patient Monitoring programs. We are seeking an experienced, compassionate, and highly organized Care Coordinator to support patients with chronic conditions through proactive monthly outreach, care plan management, and remote monitoring services.

Position Summary

The CCM/RPM Care Coordinator will engage patients regularly to improve health outcomes, reduce hospitalizations, increase medication adherence, and close gaps in care. This role requires strong communication, documentation, and patient engagement skills.

Responsibilities

Chronic Care Management

  • Complete monthly patient outreach calls
  • Update individualized care plans
  • Review medications, barriers to care, and patient concerns
  • Coordinate referrals, appointments, labs, and preventive screenings
  • Assist with transitions of care after ER or hospital visits

Remote Patient Monitoring

  • Monitor incoming vitals (BP, glucose, pulse ox, weight, etc.)
  • Escalate abnormal readings to provider staff promptly
  • Educate patients on device use and compliance
  • Improve patient engagement with monitoring programs

Documentation & Compliance

  • Accurate documentation in EMR/EHR
  • Maintain HIPAA compliance
  • Track monthly time requirements for billing compliance
  • Assist with workflow efficiency and quality reporting

Qualifications

Required

  • Healthcare coordination, medical assistant, case management, nursing support, CNA, or call-center healthcare experience
  • Strong communication skills
  • Ability to work independently remotely
  • Experience with EMR/EHR systems
  • Reliable computer/internet

Preferred

  • Experience with CCM or RPM programs
  • Knowledge of chronic disease management
  • Familiarity with Medicare populations
  • eClinicalWorks, Tebra, Epic, or similar EMR experience

Compensation

Competitive pay growth opportunities bonuses based on performance

Pay: $10.00 - $30.00 per hour
